Every effort has been made to insure the accuracy of all information. The Marion County Council on Aging is not responsible for any errors. OHIO DISTRICT 5 AREA AGENCY ON AGING:
The Ohio District 5 Area Agency on Aging (AAA) is a private non-profit agency designated by the State of Ohio to administer State and Federal funds, as well as other grant funds. Located in Mansfield, the AAA covers nine counties in north central Ohio, including Marion County. Below is a summary of their programs. The AAA can be contacted by calling 1-800-860-5799.
PASSPORT: PASSPORT provides an alternative to nursing home placement through coordinated in-home services to Medicaid eligible older adults, age 60 and over. PASSPORT covers personal care, homemaker, respite, home-delivered meals, chore, medical transportation, adult day care, home modification, durable medical equipment, and more. Case managers are used to coordinate services.
LONG TERM CARE CONSULTATION: This service is available to anyone seeking long term care, the opportunity to ask questions about their options. Can be by phone or in-home. Previously known as Care Choice Ohio.
CARE COORDINATION: Care Coordination helps to locate, activate and coordinate in-home services for frail elderly age 60 and over, who are at risk of needing institutional care. The Care Manager links consumers and families with available local resources and can purchase homemaker, personal care, or respite services for them with available funding.
LONG TERM CARE OMBUDSMAN: The Long Term Care Ombudsman provides a voice for consumers of long-term care services in nursing homes, county homes, residential care facilities, adult care facilities, adult foster homes and in private residences, regardless of age. Anyone may call or write the Ombudsman to talk about a concern they may have.
NATIONAL FAMILY CAREGIVER SUPPORT PROGRAM: This program can help if you find yourself in the role of caregiver. You can receive a no-cost caregiver newsletter, as well as Information & Assistance.
